New Member Education Forms

• Anti-Hazing Commitment Form: As of Fall 2024, this form will now be sent via DocuSign to Chapter Presidents, New Member Educators, and Chapter Advisors. For questions, please email uf-greeks@ufl.edu.
• Checklist for New Member Process: MGC and NPHC Only
• Interest Meeting Sign-In Form: MGC and NPHC only
• Initiation Report Form: To be completed by UF Greek organizations that are taking new members in Fall 2024. This report must be submitted within 24 hours of your organization’s initiation process when new members make the transition into active membership.
• New Member Process Intent: MGC and NPHC only
• New Member Report Form (Fall 2024): To be completed by UF Greek organizations that are taking new members in Fall 2024 within 24 hours after recruitment/intake concludes.
