New Member Education Forms
- Anti-Hazing Commitment Form: As of Fall 2024, this form will now be sent via DocuSign to Chapter Presidents, New Member Educators, and Chapter Advisors. For questions, please email uf-greeks@ufl.edu.

- New Member Report Form (Fall 2025): To be completed by UF Greek organizations that are taking new members in Fall 2025 within 24 hours after recruitment/intake concludes (a.k.a. new members who accepted bids or formal invitations to join).
- New Member Roster Template – please download a copy to edit. The form will require you to upload your new members as an excel sheet.
- Initiation Report Form (Fall 2025): To be completed by UF Greek organizations that are taking new members in Fall 2025. This report must be submitted within 24 hours of your organization’s initiation process when new members make the transition into active membership (a.k.a. new members who have been initiated)
